Early Supported Discharge & Stroke Rehabilitation
Overview of the service
The Early Supported Discharge (ESD) and Stroke Rehabilitation Team aims to:
- Enable and coordinate an earlier discharge for stroke patients from hospital to the community.
- Provide a short period of intensive specialist rehabilitation in the home environment.
- Work towards realistic patient led goals to maximise independence.
Some patients require on-going hospital treatment and may not be suitable for the service, but this will be assessed by your hospital team and the ESD therapists.
Who can access this service?
This service is available if:
- You have had a stroke.
- You have a GP within the Sutton CCG area.
- You are able to get from a bed to a chair
What can you expect from the service?
Once the hospital team have identified that you may be appropriate, they will coordinate a discharge date with the community team and organise a package of care with Social Services if required. Once home, one of the ESD stroke therapists will visit within a few days of discharge. The team provides regular therapy to support you at home and help you regain independence. Following a full assessment, a personalised rehabilitation plan will be developed with you to help you identify goals or aims. As part of this rehabilitation plan, it is expected that you will actively practise exercises and techniques outside of therapy sessions to work towards your own rehabilitation. After your initial few weeks at home, the therapists will begin to visit less often to allow you more time to practise and return to day-to-day activities.
