Sutton Health and Care at Home
Sutton Health and Care at Home is an innovative model of care, developed and delivered jointly by local health and social care organisations to deliver health and social care at home and in the community.
We do this by working together to respond promptly to people in need to reduce unnecessary hospital admissions. This includes access to diagnostic tests and diagnosis (as required) and ensuring you have the appropriate support at home.
Where a hospital stay is required, we will work with you to get you home as soon as you are well enough, with the necessary health and care package in place.
Who is in the team?
You may see a variety of staff, including nurses, doctors, social workers, therapists, GPs, reablement (staff who will help you regain a level of independence to remain safely at home) and home care staff. Our team have years of experience caring for patients with complex health and care needs.
Who can access the service?
The service is for people aged 18 or over with complex health and care needs, who live in the London Borough of Sutton or who are registered with a Sutton GP.
You may be referred to the service by any health or social care professional. You can also self-refer if you have been under the care of the service before.
What you can expect from us
Working with local people, we have developed a series of statements which we would like every person who accesses Sutton Health and Care at Home to be able to say yes to - we call these ‘I-statements’. These are:

Sutton Health and Care at Home Hospital Hub
Based at St Helier Hospital, our Hospital Hub Team, which consists of therapists, nurses, social workers, and GP, work together and with hospital services to identify patients in the Emergency Department, Acute Medical Unit and on the wards, who’s condition could be safely managed at home with the right support, reducing length of stay and unnecessary admissions.
