The online booking form is our preferred method of contact for the service.
If you cannot complete the online form or have additional needs, you can make an appointment via Sutton Health and Care’s Single Point of Access on 020 8296 3910. Your call will be dealt with confidentially.
Please only use one booking method and not both as it results in double appointments, which reduces availability for others.
